Opening a clinic, medical centre or pharmacy in Dubai requires more than an economic licence. The facility category, approved healthcare services, premises, design, licensed professionals and operating systems must all satisfy Dubai Health Authority requirements before patients can be treated.

What is a Dubai healthcare facility licence?

A healthcare facility licence is the DHA authorisation attached to an approved healthcare establishment. DHA lists eligible facility types including hospitals, clinics, medical centres, pharmacies and other regulated healthcare facilities.

The economic licence establishes the commercial entity. It does not, by itself, authorise the facility to receive patients, dispense medicine or provide clinical services. The healthcare facility licence must be activated after the applicable design, professional, inspection and operational requirements are completed.

Choose the correct facility category

The facility category determines the permitted services, room requirements, equipment, staffing and professional mix. Common models include:

• Clinic: a focused outpatient facility providing one or more approved clinical services.
• Medical centre or polyclinic: a larger outpatient model with multiple approved specialties and shared support areas.
• Pharmacy: a medicine-dispensing establishment subject to pharmacy-specific layout, professional and operational controls.
• Diagnostic facility: a laboratory or diagnostic centre whose requirements depend on its tests, equipment and referral model.
• Hospital or day-surgery facility: a higher-complexity category with extensive clinical, engineering and governance standards.

Do not select a category only because its setup appears cheaper. The approved category and specialties must match the services the business will actually provide.

What is Sheryan?

Sheryan is DHA's digital licensing gateway for healthcare facilities and professionals. New-facility applications, licence activation and professional licensing are managed through the relevant Sheryan services.

DHA's new-facility service can issue an inactive facility licence after the preliminary application is approved. The facility cannot begin operating until the licence is activated and all activation conditions have been met.

Economic licence vs DHA facility licence

Documents for the initial facility application

DHA's published new-facility requirements identify documents such as:

• passport copies of the owners or partners;
• Emirates ID copies where applicable;
• a facility proposal or business plan;
• an engineering layout prepared by a competent design or engineering company; and
• the economic licence if it is already available.

The economic licence may not be available at the first DHA application stage, but an active commercial licence is required before final activation. DHA can request additional category-specific documents.

Prepare a practical facility proposal

The proposal should explain the ownership, facility type, clinical services, patient profile, location, staffing plan, equipment, operating model and implementation timetable. It should be consistent with the activity requested on the economic licence and the specialties shown on the layout.

A proposal that describes a simple clinic while the drawings show several specialties, a pharmacy or diagnostic equipment can cause questions and delays.

Layout and DHA Health Facility Guidelines

The layout should be designed around the relevant DHA Health Facility Guidelines. Depending on the facility, it may need to address reception and waiting areas, consultation and treatment rooms, accessibility, infection control, clean and dirty flows, medication storage, waste management, staff areas, emergency provisions, medical gases and equipment clearances.

Before committing to premises, verify:

• the property's approved use and landlord or developer conditions;
• whether the unit can accommodate the required clinical rooms and circulation;
• accessibility, lifts, parking and ambulance access where relevant;
• electrical load, ventilation, plumbing, drainage and medical-gas requirements;
• fire and life-safety requirements; and
• whether structural or external modifications are permitted.

A low-rent unit can become costly if it cannot meet the technical standard. Layout review should therefore begin before a long lease or major fit-out commitment.

Step-by-step healthcare facility setup

1. Define the facility category and services. List every specialty, procedure, diagnostic service and pharmacy function.
2. Select the legal form and jurisdiction. Align the economic activity with the proposed healthcare operation.
3. Screen the premises. Check planning, building and DHA design feasibility.
4. Prepare the proposal and engineering layout.
5. Apply through Sheryan for the new facility licence.
6. Obtain the inactive facility licence. This is not permission to treat patients.
7. Complete commercial incorporation and the active economic licence.
8. Carry out the approved fit-out. Coordinate building, safety and other technical approvals.
9. Recruit and activate eligible healthcare professionals.
10. Appoint the medical director.
11. Prepare systems, policies and evidence for inspection.
12. Pass the applicable DHA inspection and activate the facility licence.

Medical director and professional licences

The facility needs an eligible medical director and appropriately licensed healthcare professionals for its approved services. Each professional follows a separate eligibility, registration and licensing process. A facility approval does not make an unlicensed doctor, nurse, pharmacist or allied-health professional eligible to practise.

Build the staffing plan early. A specialist service cannot normally be activated merely because the room and equipment are ready; the relevant professional licensing and facility association must also be completed.

Inspection and licence activation

Before activation, the facility should be ready to demonstrate that its physical premises, equipment, professionals and operating arrangements match the approved category. DHA's activation information refers to an active commercial licence, activation of healthcare professionals and appointment of a medical director among the required steps.

Inspection preparation can include:

• approved drawings and completed rooms;
• installed, maintained and calibrated equipment;
• professional licences and duty arrangements;
• infection-prevention and emergency policies;
• medicine, consumable and waste controls;
• patient records, consent and privacy procedures; and
• facility signage and required safety evidence.

Electronic records and NABIDH

DHA's facility requirements refer to electronic medical records compliant with applicable NABIDH standards and minimum data requirements. The selected clinical system should therefore be assessed before launch rather than treated as an ordinary appointment calendar.

Access controls, audit trails, backups, clinical documentation, consent and patient-data handling should form part of the operating plan.

How much does a healthcare facility cost?

There is no reliable single price for every clinic or medical centre. The total depends on the facility category, specialties, premises, room count, equipment and staffing. Budget for:

• economic licensing and incorporation;
• DHA application, facility and inspection fees;
• rent, deposits and tenancy registration;
• healthcare design, engineering and authority submissions;
• fit-out, fire safety, accessibility and building works;
• medical equipment, furniture and calibration;
• professional licensing, recruitment, visas and insurance;
• electronic medical record and cybersecurity systems;
• medicines, consumables, waste and maintenance contracts; and
• working capital and annual renewals.

DHA publishes service-level fees by facility category, but these are not the full project cost. Obtain a category-specific commercial and technical budget.

Common setup mistakes

• signing a long lease before confirming healthcare-design feasibility;
• confusing the economic licence with permission to treat patients;
• starting fit-out before the healthcare layout is properly reviewed;
• requesting a facility category that does not match the intended services;
• recruiting professionals without checking DHA eligibility;
• leaving the medical director, EMR or operating policies until the final inspection;
• assuming a pharmacy follows the same requirements as a general clinic; and
• budgeting only for licence fees and excluding fit-out, equipment and staffing.
